_____Nebo Chorus and Dance:     Mondays 2:15 to 4:00:  3rd, 4th, and 5th Graders
                                                          First Practice:  Monday, September 14th
                                                          Nebo Chorus and Dance is a free program.  No ASP Fees will be charged
                                                          for students picked up by 4:15.)

_____ASP Drama:                      4th and 5th Grade Auditions:  Monday, November 2nd, 3:00 to 5:00 PM
                                                      (Membership by audition – limited to 30 students)
                                                      (ASP fees of $6.00 per day must be paid for this program.)

Parent signature indicates consent for above child to be photographed and/or videotaped while in rehearsals or concerts and authorizes use of these pictures without charge for publicity purposes related to the promotion of the Nebo Chorus.  I also release Nebo Elementary School from all claims of any interest of any kind of image on behalf of both the undersigned and the child named above.
